This Custom Search Form looks very similar to the form used to enter DTR records - because it has been designed to make it easy to find a record that already exists-during development. In practice the user would not be present with a "million options" because it would create too much choice and confusion - Data from this form submission is Not saved i.e Settings=>OnSubmit - "Do not store entries submitted from this form".
